Guide to the Power of Attorney Form for Exercising Personal Information Rights

This is the power of attorney form to be submitted when a representative, rather than the data subject, exercises rights regarding personal information, such as access, correction or deletion, or suspension of processing, in accordance with the Privacy Policy.
Please download and complete the attached power of attorney form, then have it signed or stamped by the person granting authorization.
Please submit the completed power of attorney to the department below by email or in writing.
To protect personal information, the Company may verify whether the person exercising the rights is a legitimate representative and, if necessary, may request additional documents to verify the identity of the data subject or the representative.
Submission of the power of attorney : Email to the Management Planning Division (encell@encellinc.com) or in writing as instructed by the responsible department
